I've had this happen twice this week. Once with a spy-op and once with a self-op from the Advisor page.

In both cases I was 100% sure of the action (still had it on the screen in EE) and it didn't make it to Boxcar....

boxcar is bugged, country db wasn't updated last time I checked (few days ago) so deads were allive etc.
